An annual inspection is only as valid as the living privilege behind the signature. In NTSB case CEN24LA286, a Piper Malibu crashed after an annual signed with the A&P certificate number of a mechanic who had been dead for nearly 17 years. This short follow-up to our Project Icarus analysis moves from the pilot's licence to the mechanic's certificate and inspection authorization. It covers what the NTSB established, what makes an IA signature legally alive, and a checklist for shops and buyers who accept signed annuals.
What the NTSB report establishes
Everything in this section comes from the NTSB final report CEN24LA286, published April 2, 2025, and its public docket.
On July 28, 2024, a Piper PA-46-310P Malibu, registration N4646G, suffered a total loss of engine power about 25 miles from its destination. The pilot landed in a grass field near Radisson, Wisconsin, uninjured; the airplane was substantially damaged. On-site examination found a cap missing from the fuel pressure test port, with blue fuel staining around it. Engine monitor data showed fuel flow spiking to about 80 gallons per hour, the maximum the system could record, just before the engine quit.
An annual inspection had been performed on June 14, 2024, about six weeks before the accident. The work included new fuel injectors. The engine manufacturer's maintenance manual requires a fuel system operational check after injector replacement: test equipment connects to that port, and the cap must be tightened and torqued afterward. The NTSB determined the probable cause: the non-certificated mechanic's failure to properly tighten and torque the test port cap, which caused the total loss of power and the forced landing.
Then the report states what makes this case exceptional. The individual who performed the most recent annual inspections held no airframe and powerplant certificate and no inspection authorization. He signed the annuals and returned the airplane to service using the A&P certificate number of his deceased father, who had held an inspection authorization. The father died on June 25, 2007 — nearly 17 years before the 2024 annual. The report's own words: the individual "fraudulently represented himself as a certificated mechanic with inspection authorization and the appropriate knowledge, training, and experience by signing off the annual inspections and returning the airplane to service."
Two things the report does not establish. The son is not named, and the report says nothing about criminal or civil consequences. Whether the aircraft owner knew anything is also not in the record. Press coverage adds color, clearly marked as reported context. General Aviation News reported on July 22, 2026 that the father's certificate dated from 1971 and his last IA renewal from 1984, and that the son apparently ran the maintenance operation for close to two decades. Those details come from the docket and the columnist's reading, not from the probable-cause findings. Columnist William E. Dubois concluded in his Deadbeats column: "we need to fall back on 'trust but verify'", with "VERIFY in all caps" when dealing with a new-to-you mechanic.
What makes an IA signature legally alive
Here is the structural point. An A&P certificate number written in a logbook proves that a certificate exists. It proves nothing about whether the privilege behind it was alive on the date of the signature. Under 14 CFR Part 65, living validity is a chain of four conditions, and every link can fail independently.
- The certificate. Under 14 CFR 65.15, a mechanic certificate is effective until surrendered, suspended, or revoked. It has no expiry date — which is exactly why its existence proves so little. Under 14 CFR 65.83, privileges also require recent experience: at least six months of relevant work or supervision within the preceding 24 months.
- The inspection authorization. Only an IA holder (or an appropriately rated repair station) may perform an annual and approve the aircraft for return to service (14 CFR 65.95) — with one narrow exception: a manufacturer may perform an annual on aircraft it manufactured under a type or production certificate, and approve the return to service (14 CFR 43.3(j)(3) and 43.7(d)). Eligibility under 14 CFR 65.91 requires a currently effective mechanic certificate with both ratings, in effect for at least three years. The applicant also needs two years of active engagement, a fixed base of operations, and a written test.
- The currency clock. Under 14 CFR 65.92, every IA expires on March 31 of each odd-numbered year. Renewal under 14 CFR 65.93 requires qualifying activity in each year of the two-year cycle: annuals performed, major repairs, a refresher course or an oral test. An IA that was valid in 1984 was dead paper in 2024.
- The human. The privileges are personal. Section 65.95(b) requires the holder to keep the authorization available for inspection by the aircraft owner whenever he exercises its privileges. A certificate number can be copied from an old logbook; a living holder who can show a current IA cannot be copied.
A checklist for accepting a signed annual
You run a shop, manage an aircraft, or review logbooks in a pre-purchase. These checks are pass-or-quarantine, and none requires new software.
- Read the signature block. Every annual entry carries a certificate number and a date. No number, no entry — reject it on sight.
- Look the number up. The FAA's online Airmen Registry confirms whether a mechanic certificate exists, under which name, and when it was issued. Match the logbook name to the registry name.
- Check the human, not just the paper. Government photo identification against the registry name. In Radisson, the signer and the certificate holder were two different people, one of them dead for nearly 17 years.
- Ask to see the IA. Section 65.95(b) obliges the holder to keep the authorization available for inspection by the aircraft owner whenever he exercises its privileges. That is a regulatory duty, not a courtesy. Then check its currency against the date of the annual: March 31 of odd-numbered years is the hard boundary.
- Test the recent-experience link. A fixed base of operations and active engagement in maintaining aircraft are IA eligibility conditions. Read section 65.91(c) precisely: the fixed base is where the holder can be located in person or by telephone during a normal working week, and it need not be the place where he exercises his inspection authority. The equipment, facilities and inspection data must be available to him — not necessarily owned or housed in his own shop. A "mechanic" with no reachable base, no access to the necessary equipment and no traceable activity does not by itself prove an invalid authorization — but it is precisely the signal that calls for a currency check with the FAA before you trust the entry (sections 65.91, 65.93 and 65.95).
- Mind the arithmetic of dates. Per the General Aviation News account, the father's certificate was issued in 1971. A certificate older than its apparent holder is a red flag you can spot in ten seconds.
- In a pre-purchase, re-run the chain on every annual in the logs. Not the last one — all of them. A single fraudulent entry contaminates the airworthiness record built on it.
- Quarantine on one failure. Do not confront the signer. Contact the responsible Flight Standards office, and treat the affected entries as unverified until the FAA resolves them.
The limits of what this proves
Three limits keep this analysis honest.
- The NTSB report is a factual record, not a verdict. It establishes what was found and what the probable cause was. It does not adjudicate criminal guilt, name the son, or say what consequences followed. Anything beyond the report — the 1971 certificate date, the 1984 IA renewal, the length of the operation — is press-reported context, and we have labeled it as such.
- Registry checks confirm existence, not liveness. The Airmen Registry tells you a certificate exists. IA currency at a historical date and the identity of the person who physically signed are separate checks, and they are procedural, not cryptographic.
- Anchoring fixes alteration, not impersonation. A hash-anchored logbook entry proves the entry was not changed after the fact. It does not prove the signer's privilege was alive on the signing date. That link still requires the authority-facing checks above — which is why the checklist comes first and the tooling second.
